Making and keeping appointments
Many of your appointments are scheduled weeks in advance and we make every effort to confirm these 1-2 days prior.
If you are unable to attend your scheduled appointment we ask that you contact our surgery with no less that 48hrs notice so that we may accommodate another patient.
Medical History Questionnaire
Should you wish to complete your medical history questionnaire prior to your initial appointment with us, we have provided two easy ways for you to do this.
Click here to print a copy of the medical history questionnaire to complete at your leisure. Once complete, you can fax it to us on 02 4578 4121 or simply bring it with you to your appointment.
A fast and easy way to get your form to us, click here and complete your medical history questionnaire online. Once you have completed this, it will prompt you to use your email account to send it. The form will be attached to an email and forwarded to our office for you to sign once you arrive.
Payment terms
Our practice requires payment on the day of treatment. Payment can be made by: Cash, Credit card or EFTPOS. We also provide the HICAPS facility to enable you to claim before paying your part of the account. We do not accept personal or business cheques.
